Our negative paps are never read by the pathologist but, rather the cytologist. Can we bill 88142 with a TC?

The concept of a professional / technical component does not apply to CPT 88142 (G0123 - HCPCS) so you would not want to add modifier TC. There are separate codes available for the pathologist actually reads/interprets the pap smear (88141 / G0124).....

The "National Physician Fee Schedule Relative Value File" on the CMS website contains a column that provides a PCTC indicator that may be helpful.
