Need help with cardiac monitor

Our physician is now using a cardiac monitor (extended holter) in the office. They are cat III codes of 0295T & 0298T (more than 48hr and up to 21 days). Would anyone know where I could locate a reimbursement guide on these?
 
Currently CMS does not have and LCD for JEB, but there is an article ID#A57476 for contractor Wisconsin Physicians Service Insurance Corporation , but it discuses procedures you may use 93224-93227.
