Surgeon did a transoral incisionless fundoplication for GERD. Would the anesthesia code be 00500 or 00740? 00500 doesn't seem right, base units are high at 15. Any thoughts appreciated.
An intraoperative EGD was performed after induction of anesthesia. The LES appeared to be lax. The esophageal portion of the GE junction was inspected no evidence of erosions/ulcers. We then placed the EsophyX device on the endoscope and placed this in the esophagus and advanced it into the stomach slowly. Retroflexion views of the EsophyX device showed application of the fasteners was done in a safe and a circumferential manner as prescribed. Approximately 16 fasteners were used because of occasional misfires. We began first by performing a wrap in the anterior position and then in the posterior position creating a 3 cm lip. The device was then extracted and the esophagogastroscope then reinserted. There was no significant bleeding or complications. The esophagus appeared intact with only mild traction type injury to the mucosa. The scope was removed after desufflation of the stomach and the bite block removed.
