hi im having a hard time billing the 99406-99407 in conjunction with the 99344
this is how i code:
99344 25 modifier
94760
99406 or 99407

but medicare denied because the procedure code is inconsistent with the place of service . please help . i use POS 12 , is this the right POS? when billing 99406-99407? :o

Where is the patient housed? Is the patient in a SNF, at a personal residence, or assisted living?
