I do Outpt Coding for a physicains office.

I have a Mercy Care (AHCCCS) pt..


Procedure: Harvesting of right greater saphenous vein.
Reversed interposition vein graft to left brachiocephalic fistula
Superficialization of fistula

Need CPT help--- I am going to use 36832 -- for Revison, of fistula .

I am not sure if I need to code for the Harvesting of vein (35572 ??)
Also I am unable to find a code for (Superficialization of fistula).
 
I would definately bill the harvesting but I would use 37799 (it is an unlisted) because 35572 cannot be used with a revision, 36832, according to CPT notes.
