provider removed multiple splinters from right hand third, fourth, and fifth digit with mosquito hemostat. do I bill only 10120 once? or separately for each removal?

If 10120 represents the work performed on each removal, then you may bill it three times. Don't forget your F7, F8 and F9 modifiers as well as either -59 or XS to show the F/B was removed from separate anatomical locations.
