I need to submit a claim to Medicare for the following cpt 64774 excision of neuroma, cutaneous nerve with add on +64787 implantation of nerve end into bone or muscle(list seperately in addition to neuroma excision with 64784 excision of neuroma, major peripheral nerve with add on +64787 as well. How do I submit the multiple add on without it denying as a duplicate, do I need a 59 modifier?

Add on codes should not have to have a modifier. Don't use -59 modifier with add on codes, but you can use -51.
