Wiki Modifier required for Humana

I recently billed out the following codes to Humana NPOS : CPT 98941 and G0283. I recieved a denial back stating the missing modifier is required. Has any one else had this issue?

Per AAPC Coder 98941 should be billed out with AT modifier.

AT	Acute treatment (this modifier should be used when reporting service 98940, 98941, 98942)

As far as the G0283, maybe one of these?

GO	Services delivered under an outpatient occupational therapy plan of care
GP	Services delivered under an outpatient physical therapy plan of care
KX	Requirements specified in the medical policy have been met
