what modifer is used with 76496 and 76499?
is it different when billing just 76496 and 76499 on separate days ?

Hi - There aren't Medicare or Medicaid CCI edits for this code pair. They're both unlisted procedure codes, so your payer should have a policy on how to report them - the additional documentation requirements should allow you to explain the circumstances similar to the way a modifier does.
