Are there any payers who require the AT modifier with 58661 (or any 58000 series of CPT codes) in place of a consent form?
 
Hi - I found a reference to AT in a 2016 Illinois healthcare handbook, but it doesn't address your question clearly (plus rules may have changed since then): "Performance of a procedure that renders a participant sterile, but that was not performed with the intent to sterilize the participant (e.g., removal of a portion of a fallopian tube due to an ectopic pregnancy) must be submitted with the appropriate surgical CPT code and Acute Treatment modifier “AT."" https://www.illinois.gov/hfs/SiteCollectionDocuments/PractitionersissuedOctober2016Handbook.pdf

The official descriptor for AT says it's for chiropractic codes 98940-98942, so use for other purposes does seem payer specific and should be stated in written policy. Perhaps members in the ob-gyn forum can help. Best of luck!
