68761 is reported with modifiers E1, E2, E3 and E4 as the case may be.

Depending on your insurance carrier, of course! Some don't recognize the HCPCS codes submitted as modifiers.

Punctul Plugs

You will use 68761 twice but instead using RT or LT will use E1,E2,E3, and E4 look at your Dr's notes and check what was done.
You can also use A4263 which is supply but pattern of E1,E2,E3 and E4 will remail the same.
I submit claim with same pattern never rejected.
