Can anyone tell me if there is a code for this? or is it inclusive? All the note states is "The patient's mouth was then opened with a mouth gag. Using a mirror, the adenoidal pad was inspected and noted to be normal. The mouth gag was removed."

92502 but since it was during tube placement, the 92502 would be 'bundled'. The 92502 includes ears, nose and larynx. (If the patient's nose wasn't inspected, then you would use a -52 for "reduced service".) Since placing the tubes includes inspecting the ears, the separate "inspection" code would be denied as bundled, since that also includes ears.
