Patient had displaced fifth metacarpal fracture and a third metacarpal fracture.
Fifth metacarpal fracture was manipulated in office. Third metacarpal was not manipulated.
My posting would be 26605 for the fifth metacarpal fracture. For third metacarpal I will code 26600-59.
Am I correct in this coding. Or can I only code the 26605?
 
It depends on the insurance. Per Medicare NCCI you can only bill for the highest procedure treated with a single cast. However, other payers may not follow Medicare guidelines. Review the Medicare NCCI info. It's very helpful for situations like this.
Under # 16. Additionally, if a single cast, strapping, or splint treats multiple fractures without manipulation in addition to one or more fracture(s) with manipulation, a closed fracture without manipulation CPT code shall not be reported separately.
