G0403 EKG, routine w/12 leads;performed as a screening for the initial preventive physical exam

G0404 EKG, routine w/12 leads; tracing only, w/o interpretation and report, performed as a screening for the initial preventive physical exam

G0405 EKG, routine w/12 leads; interpretation and report only, performed as a screening for the initial preventive physical exam
