What CPT code should be used when a physician provides a medical clearance exam at a behavioral health facility during a patient’s admission?
 
What does the note indicate that the provider did? Was this a psychiatrist? Was it a medical exam or a psychiatric exam? Was this the attending provider, or a consultation?

Either an initial hospital visit or a psychiatric diagnostic interview (without seeing the documentation), but in this case, the location of the service has little bearing on the code assigned. You code what was performed.
 
I'm auditing a family practice provider, who preforms medical diagnosis for patient during their admission at this behavioral health facility. Psychiatrist has billed either 90791 or 90792 with a psych diagnosis. The family practice provider is billing 99221-99223 with diagnosis such as Obesity, acne, etc. the admitting H&P was completed by and APN and this provider is signing off the notes and billing for service. I want to say that the initial visits can only be billed by the admitting provider who's diagnosis on the billing was relevant to behavioral health service. Thank you. Appreciate your help.
