Wiki Mediblue (BC Medicare Advantage) Denial Question

We have been receiving denials from Mediblue since the beginning of the month of G60 16. Incorrect CMS claim form was submitted. We haven't changed the way we submit. Anyone else getting this denial? I called Mediblue to see if there was a glitch on their side and they said no.
