Wiki Medi-cal and L2300 Code

Any help would be appreciated; I have billed the L2300 code w/o a Modifier, with a LT, RT & LTRT; to no avail it keeps being denied as "Modifier not Present". I have even called Medi-cal and they can't understand why even thought thier manual states "their is an exception of a list of codes that can be billed without modifiers".

Does anyone in the O&P industry have any ideas or have had the same problem?:confused:
 
You are correct that L2300 should not be billed with RT or LT. Was an authorization request done? If so, what codes/modifiers are listed? Do they match the claim form? Perhaps it's a DME-specific modifier they're looking for? :confused:
