I am coding for 11046 the additional 20sq cm the physician documents one wound 18x24x0.3 and another wound of 15x24x0.3 will this make cpt code 11046 with 38 units 11043 with 1 unit

Actually you're short one unit.

The total size of the debridement is 792 sq cm. 11043 is for the first 20 sq cm. leaving 772 sq cm. If you code 38 units you're only accounting for 760 sq cm. for a total of 780 sq cm.

Be advised there is an MUE of 4 (maybe 5) for these codes so this will likely be denied. Make sure the provider's documentation (preferably with pictures) supports a debridement this large.
