Wiki Mac w/block and u/s guidance

In the last Anesthesia Coding Alert it said that when MAC or Regional Anesthesia is given the block is included but if u/s guidance is used, you can bill for that. What diagnosis would you then use?

The diagnosis would be selected based on the reason documented in the medical record for the block such as acute post operative pain 338.18.
 
ultrasound dx

Diagnosis 338.18 / G89.18 doesn't seems to be an appropriate to meet the requirement of this question. I believe no separate diagnosis would be coded for ultrasound. Howerver ultrasound should be billed.
