HI, Need help with correct coding for a Lumbar fusion surgery (ALIF +PLF)
3 surgeons are involved - A- Orthopedic, B- approach surgeon, C- Orthopaedic

ALIF - A+ B are co- surgeon, C is assist
A : 22558 - 62
B : 22558 - 62
C : 22558 - 80 (assists with the cage + instrumentation ) - is an assist allowed when 2 co-surgeons are already billing the primary code?

PLIF
A +C are Co surgeon
22612 - 62
22612 - 62
Would Mod- 59 apply for 22612?
