For Medicare part B, how can i bill for Lucentis injection when it was done on both eye. Thank you.
 
You should be able to bill the injections as 67028-50. Some carriers want: 67028 RT and 67028 LT

Bill for the Lucentis with J2778 and then show the number of units used based upon the unit dose specified in the HCPCS description.
