Don't use the size from the pathology report, the specimen is trimmed before submitting to the lab. If the OP report said 6.4x4.7x1.8cm for the size of excision, I usually pick the big # for coding.

He didn’t give size in op note so will have to use the minimum- us this the right code range for sub muscular ?

I would ask/query dr for the size of excision clarification, since the # looks big in pathology report. 23073 Vs 73076 depends on clarification from surgeon.
