I need advice on how to code this procedure. Physician completed a radical nephrectomy laparoscopically, but when he was performing the ureterectomy portion, he had to convert to an open procedure. I cannot bill 50545 and 50650 together according to CCI edits. I know I would normally bill open when a procedure is converted to open, but would that be true in this case since most of the procedure was completed before converting to open? Is the ureterectomy included in 50545?
