Notes: 3 view left foot x-ray revealed an acute, mildly commuted and displaced fracture involving the base of the left fifth metatarsal. Ace wrap applied to left foot and ankle. patient was placed in an orthopedic walking boot. Patient being referred to orthopedic surgeon due to displaced fracture

Ace wrap applied to left foot and ankle and placed in an orthopedic walking boot by this nurse, patient tolerated well. Education provided to patient regarding donning, doffing and how to check extremity for adequate circulation. Patient vocalized understanding.
Above is the note that was documented for the application of the walking boot. Based on this what CPT codes would you use for the application of the boot? Strapping and/or splinting or is it included in the e/m?
