Good afternoon,

An LCD came out in February that said MCR will not pay for the ki67 on breast panels because the way of reading the test is not standardized across labs. However, there was no utilization ceiling, and we are not sure what to do if it is not a standing order but rather being requested by an oncologist. Is it then reasonable and necessary for patient care, and should I charge for it?
