Wiki KD modifier on drug for pain pump

Confused on Medicare guidelines for KD modifier for drug refills on implantable pain pumps. We use Baclofen (j0475) and was told to use the KD modifier but it states to use on J3490. Anyone used KD modifier, any suggestions? Thanks Nancy
