How does everyone bill Avastin to UHC? Per their policy, they want J9035, but the drug amount for the code is much higher than what is administered.
 
Oh my gosh, we just started injections this week. So far they have been Medicare. I thought regardless, you bill 1 unit even though J9035 is 10mg. Did you ever find the answer?
 
If a drug comes in a "one time use" vial or ampule, the whole amount is charged. I read somewhere that some drugs need the amount wasted shown too, but I don't know where I read that. :/
 
Medicare: Effective for dates of service 9/1/19 and after, the allowed amount for intraocular bevacizumab (Avastin) has been updated to $94.00 per dose.

Notice it does not say "per milligram" or "per milliliter". It is one unit.
