Hi All! We have been using an FPU2 modifier on all J8499 hcpcs for years. Now we've received two denials from Anthem HealthKeepers Plus for inappropriate modifier. I can't find an alternate I can try, or should I try to omit it all together and send in a corrected claim? :confused: TIA.
