I have several questions regarding IUI prep and semen analysis.

89260 and 89261 indicate they include semen analysis. Is that semen analysis both before and after? Or only one semen analysis? If only one, what code is used for the second analysis?

Semen analysis codes (89310, 89320, 89321, 89322) cannot be billed under a female, however the analysis is being done for treatment of the female. How is that billed?

I think I am following your question.

The diagnosis code of z31.89 or z31.81 should be used. When we bill IUI's with absolute male factor, the codes we bill are 58322 and 89261...as 89261 states it is used for diagnosis OR use in insemination.
