I am trying to bill 67403 lt and 67228 51 rt to medicaid. I know i cant use the lt or rt w/medicaid and the codes are bundled but when i do bill with the 51 the 67228 gets paid $408.85 and when i bill just the codes without the 51 mod the 67228 gets paid $820.70. Neither time does the 67043 get paid. When i try filing just the 67043 alone it does not pay at all. The allowable for the 67043 is more so i would rather have that paid. Does anyone have any advice??
