Hello,

Can anyone tell me if they bill for and receive payment for IOLs (Intraocular Lenses) with a cataract procedure? Specifically codes V2632 and V2630?

Medicare reimbursement for the standard IOL is bundled with 66984/66982, which we do in an ASC. Patients are responsible for any premium IOL not covered by medicare which would include V2788-(restore lens) & V2787-(toric) and they would need to sign an ABN form since they are not covered by medicare.

We do not bill any V2630/V2632 (only covered when services are rendered in physician office) with 66984/66982.
