A patient had surgery with 90 day global, CPT 67210 in his left eye on 6/23/2016. On 6/28/16, he had the same surgery again, 67210 with 79 modifier because it was done in his right eye. Then on 8/9/16, he had an intravitreal injection, 67028, in his left eye. For the injection, would we use a 79 modifier or a 58 modifier? Do we go by the original surgery or the latest surgery?
 
You would use the global period for the same (left) eye as the injection had nothing to do with the right eye. What was the purpose or reason for the injection? That would be the starting point to figure out how to bill for it.
