Hi , I need some help with a Medicare claims for injections of avastin my doctor use only 1.25 ml and discard the rest of the med do I have to bill the rest with modifier JW , my question is because I saw it’s mandatory to document in the claims the med discard
 
Hi - Medicare Claims Processing Manual, Chapter 17, Section 40, does say "Providers must record the discarded amounts of drugs and biologicals in the patient’s medical record" if you want payment for the discarded amount using JW. Also keep in mind that 1 unit for J9035 is 10 mg, so that will affect unit calculations.
