If there aren't any patient-specific risks associated with the injection, then yes, I would put it at low risk in MDM. At least, that's how I always took it to mean since it is a procedure - scheduling treatment for the patient's problem.
 
If you are talking about the decision to perform MBB at an E/M office visit, and the risk box of MDM; it depends on the documentation. There could possibly be a reason that it might be moderate for the patient. The documentation would have to indicate that there was a decision regarding minor surgery with identified patient or procedure risk factors. This isn't just the normal risk factors that apply to all patients and the procedure.
Poster is talking about MBB of the spine, not just an IM injection or SubQ. You can't lump all "injections" into one basket.
An example may be if the provider specifically documents they will be using a steroid and the patient has increased risk due to having DM, high blood pressure, and is obese. (This is just an example).
