I billed a 99214 a 96372 and a j3420 using mod 25 on E&M code to Medicare but my 96372 was separated and not paid. Used different dx for 99214. Help does anyone know what I did wrong?

Did the payer receive the 25 modifier on 99214? You may need to try appealing to prove the reason for the visit was separate from the reason for the B12 injection.
Check the payer's bundling guidelines - several people on the Forums lately have reported that they're having problems getting 96372 paid. Try searching for those threads and check out the discussions.

There is a recent post here that Medicare has confirmed they are incorrectly denying 96372. They should fix it within a couple of weeks. Check your Medicare webpage to see if you have to resubmit, or if they will reprocess automatically.
