My client has started new Infusion Center. Nurse has provided the following services.

1. Vancomycin 1.5 gram each for 2 days
2. Bodyguard 323 daily use
3. Maintenance of pump at Infusion
4. 1 hr and 45 mins.
5. PICC line dressing change (include picc dressing kit, bio patch).


This is what I thought to bill

1. J3370 6 units
2. ????
3. 96521
4. 96365 and 96366
5. ????

Experts, please help me to code these services correctly.
