Wiki Incomplete Screening Colonoscopy to the Sigmoid only.

I have a patient that is scheduled for a screening colonoscopy and it is their 1st colon no symptoms but when the colon is done there is poor prep and they can only make it to the sigmoid. Per the decision tree I can only bill for a 45330 or G0104 (which one is appropriate?) with the Z12.11 then poor prep dx and do I use the 33/PT modifier saying it was intended to be screening but because the prep was poor? Patients recall is repeat colon in 1 month with 2 day prep.
