I get rejections from our claim scrubber the S92.XXX is not a primary dx. I've read the guidelines and can find what I need.

S92.xxx is not a valid code. It has valid 4th 5th and 6th characters, no placeholders, and requires a 7th character.

There is no reason why an S92 code cannot be first listed. As long as you have all 7 characters. Unless you are applying the 7th character S, in that case guidelines tells us to code the late effect first. Can you giv an example of a specific one that is being denied along with all codes submitted?
