The eyelid was everted over a Desmarres retractor. A Colorado-tipped monopolar electrocautery was used to make an infratarsal incision for the full horizontal extent of the lid. A lateral canthotomy and inferior cantholysis was performed via the conjunctival approach. The dissection was carried inferiorly in a retro-orbicularis plane down to the level of the inferior orbital rim. The periosteum was opened with a monopolar cautery. Freer elevators were used to elevate the periosteum posteriorly. A portion of the lateral floor was punctured with a Freer elevator, entering the maxillary sinus. Kerrison ronguers were used to create a bony opening within the orbital floor.

Was told to use CPT 67400. Any thoughts would be appreciated. Thanks.
 
Based on your note, I would be more inclined to use 67414; 67400 is for exploration, with or without biopsy, whereas 67414 is with removal of bone for decompression (unless I'm reading more into the note than is there.)
