Has anyone had issues billing 0398T to Medicare for an assistant surgeon? Our office is billing 0398T with 82 modifier and the claims are being rejected as unprocessable/ unappealable. The Medicare physician fee schedule is showing the 82 modifier is allowed so we're kind of stuck, can't remove the modifier, can't appeal. Any info would be helpful.
