I have been billing for a DO for the past 5 years. He works as a PCP as well as an Osteopathic Specialist. He is going to start seeing patients in their homes to determine Hospice status. I am aware of the hospice modifiers of GV and GW to be used by an attending physician depending on what he is seeing the patient for but do we use the standard E and M codes for these at home visits and add the modifier? Or is there a specific list of CPT codes or criteria for at home care or hospice codes? He will be a contract provider so the place the patient is living (at home) will not be doing the billing as a facility for his services.at

There are no CPT codes specifically for home hospice patients. Your provider should use the E&M codes for home services in the range 99341-99350, with the appropriate hospice modifier if the patient has already been admitted to hospice.
