If a facility is billing for the training can the provider also bill for the training? We used to bill using the 90989 and then they stopped paying because the facility was billing and they would not pay for both. Has anything changed or is there another code that can be used in place of this for the physician to get paid for training
 
90989 is not a physician service code per the Medicare fee schedule, so can't be billed by a provider for services done in a facility. But patient education by a provider does fall under the definition of 'counseling' in the CPT E&M guidelines, so if it's medically necessary and properly documented, you can bill an E&M code for this if it meets the requirements for coding based on time.
