1. If the provider only completes a comprehensive visit/assessment during the month, can you bill for this without any other face to face visits during the month? If so, would you use 90962?

2. If the provider completes a comprehensive visit/assessment during the month and also three other face to face visits during the month, do you count the comprehensive exam as a visit? Would you use 90961 or 90960?
