Synagis Coding Help

I have a problem with diagnosis coding for Synagis 50 mg. The claim is going to Select Health/First Choice and is denying saying that the age of the patient does not correlate with the submitted diagnosis code. We're using 765.14 (other preterm infants 1,000-1,249 grams) and 765.25 (weeks of gestation 29-30 completed weeks of gestation). The Synagis is being administered to prevent RSV. The patient's DOB is 7/11/11 and the Synagis is being administered once a month starting in November. I'm using CPT 96372 but there's something I'm missing with the diagnosis.

Can anyone help?:confused:
 
prevention of rsv

for the prevention of rsv use v04.82, using the 765 category codes are only for newborn patients. If the patient has any disorders of the heart or lungs I would code this first then with the V code as a preventative.
