We had a claim deny with T78.03XD for the E/M visit. Insurance website states "DIAGNOSIS CODE INCORRECTLY CODED PER ICD10 MANUAL" I am struggling to find where, other than the ICD-10 book, where any payor, including CMS states this. Can anyone provide any insight?
