Can I get any help with the following billed CPT codes? 43753 billed with 36410 x 2, which are performed on different locations. 43753 is bundled with 36410, so I am thinking a mod 59 would be added to the first 36410 billed, but what modifier would be appropriate to bill with the second 36410 billed?
