I am having trouble getting the G0455 paid by MCR. We were told that the 45378, or the apporpriate colonoscopy biopsy code would accompany the G code. Then we were told that we have to have the modifier 59 on the G code. Now we are being told that the Inpatient Hospital setting is the wrong POS. Did I miss something somewhere?
Has anyone else been having difficulty and how did you overcome it?
