Is this CPT billable for MCR patients only ?

CPT 99374-Supervision of a patient under care of home health agency (patient not present) in home, domiciliary or equivalent environment.
Does commercial insurance pay for this code ?? Aetna, CIGNA, UHC,BS,BC

We bill all payers the G0180 code except for Medicaid. We bill Medicaid 99374. Blue Cross (non-HMO) is the only payer we have difficulties being reimbursed for G0180/99374.
