For an adverse incident report I need the correct Surgical description ICD10 codes for Implantation of wrong lens, Treatment Description ICD10 for removal and implantation of correct lens ( CPT 66986) in Ophthalmology as well as the resulting injury (None)
 
don't use H code and Z96.1 on or the other. If this is due to coming back then don't code the H59.88. Z45.59 primary dx then reason for lens and misadventures
