I am getting denials on claims for Family Pact.

I have tried
81002, QW
Q0111, 26 and
Q0111
83986, ZS
88142, 26
and 88142, QW
99000 needs modifier.

They are all denied.

Family Pact is a mystery. Any help would be appreciated. I can't wait until Dec 1st and they go to ICD9!
